If you are leaving Avon Hockey for another program, you must request a release by filling out this form. Please fill out the release form in its entirety - failure to do so will result in a delay in processing your request. The information requested is required for the purposes of completing the release form that is sent to the CHC. Requests will be processed within 2 weeks of their submission. A copy of the release will only be sent to the CHC - you will not receive a copy.
A release must be requested prior to September 1, 2020. A release will only be granted if all financial obligations to the ACFYHA have been fulfilled and any ACFYHA equipment you have borrowed has been returned.
